Brief Overview of Services/Programs |
Cambridge Home Health Care provides home health
care to individuals in the comfort of their
home. Our purpose is to deliver services that
assist individuals in reaching their optimal
level of health and independence. Our staff is
available 24 hours a day, 7 days a week to
coordinate care and instruct families on how to
function as caregivers. All personnel are bonded
and insured.
Cambridge Home Health Care coordinates home
health care for physicians, hospitals, managed
care organizations and patients of all ages. The
company is Medicare Certified and accredited by
CHAP. Services include skilled intermittent home
health care on a per hour or per visit basis.
Care is provided by Registered Nurses, Licensed
Practical Nurses, Occupational, Speech and
Physical Therapists, Medical Social Workers and
Home Health Aides. Homemaker and companion
services are also available on an hourly basis
for short or long term intervals.
Medical Surgical Nursing:
Our team of medical surgical nurses offers
primary care to patients discharged from the
hospital who require in home medical or surgical
acute nursing care in addition to care of
patients who are chronically ill.
Individualized plans of care are developed by
our health care team in coordination with the
patients family and physician. The nurse and
other medical team members serve as teachers for
the patient and their families.

Patient Family Teaching

IV Therapy
Homemaker/Home Health Aide/Companion Services
CHHC provides a full array of personalized home
care services. Our trained nursing assistants
can provide individualized care to patients in
the privacy of their homes. Services include:

Assistance with bathing (activities of daily
living)
• Meal Preparation
• Light Housekeeping Services
• "Tuck in Service" assisting patients with
bedtime preparation
• Shopping Services
• Medication Reminder
• Companion
We
have 37 offices in Ohio and Pennsylvania. Our
census includes patients from the Medicare
program, private insurance, private pay, state
waiver programs and PASSPORT programs, among
others |
